This form is intended for current HSSW foster volunteers to submit medical concerns for a foster pet in their care. Please provide as much information as possible - this will help our team provide appropriate follow up for you and your foster.

Emergency Medical Concerns

Do not use this form to communicate emergency medical concerns.

If you have an emergency, call 360.213.2633 to reach our foster team. If your call is received between 8:00 pm and 6:00 am, your call will be transferred to MedVet for after-hours support.
